A 45-year-old female had a 3 cm round tender cystic skin lesion on the right elbow for 6-month duration. The clinical diagnosis was inflamed sebaceous cyst. The cyst was excised and sent for histo-pathological examination. Spot the diagnosis.

Subcutaneous gouty nodule.
There are large areas of wispy material (tophiaceous deposits) in a fibro-collagenous matrix that shows inflammatory granulomatous reaction, which is typical of gout. [1]
